If you have private medical insurance:

  • Contact your insurance company to make sure that you are covered for treatment and to ask for a claim form
  • Check with your insurance company to see if you need a reference number before the consultation. Be aware that some policies only cover you for specifics, for example, in-patient treatment
  • Check to see if there is a "shortfall" in your cover. This means that the insurance company may not cover you for the full amount of the cost of treatment and you may be required to pay the difference
  • Complete the form and ask your GP or consultant to sign it
  • Bring your insurance form with you when you come into hospital
